Peak Community Services provides support and respite services to families who have children or adults with developmental disabilities and who have an ongoing need for supervision or care. Respite services are provided as temporary relief for the primary caregiver. Other services are available through state and federally-funded programs. We provide support: In-home: the direct support professional comes to your home to provide support. Out-of-home: support is provided in the home of the direct support professional. In the community: support is provided to participate in community activities like swimming, therapeutic horseback riding, clubs, sporting events and shopping. Individuals who qualify for the Medicaid Waiver or CHOICE programs are not charged for services. Services are provided in North Central Indiana. www.peakcommunity.com
