NCH is the largest voluntary sector provider of services of disability services for children and young people in the UK. We run over 50 projects supporting disabled children and their families. In partnership with children and young people, their families, local authorities and health trusts, we deliver a wide range of services. Disability services include: residential short breaks family-based short breaks with carers in the local community daycare services, including after-school clubs and holiday playschemes supporting housing schemes for young people advocacy services resource centres and key worker schemes offering families a single access point to services, promoting greater inter-agency co-ordination specialist play, learning and leisure facilities opportunities to access a range of social and leisure activities in the community advice and guidance for young people and their families as they make the transition to adulthood and greater independence home-based care, outreach and family support services, including support with personal care, sitting services, play sessions and palliative care www.nch.org.uk

Riverstone Families Connect has started a mentoring program for boys where experienced wood workers show them how to build thing with their hands. Peak Community Services provides support and respite services to families who have children or adults with developmental disabilities and who have an ongoing need for supervision or care. Respite services are provided as temporary relief for the primary caregiver. Other services are available through state and federally-funded programs. We provide support: In-home: the direct support professional comes to your home to provide support. Out-of-home: support is provided in the home of the direct support professional. In the community: support is provided to participate in community activities like swimming, therapeutic horseback riding, clubs, sporting events and shopping. Individuals who qualify for the Medicaid Waiver or CHOICE programs are not charged for services. Services are provided in North Central Indiana. www.peakcommunity.com
